About CMS Five-Star Ratings
The Centers for Medicare & Medicaid Services (CMS) rates every Medicare and Medicaid-certified nursing home on a scale of 1 to 5 stars across multiple dimensions:
- Overall Rating - Combined score based on all three areas below
- Health Inspection - Based on on-site inspections over the last 3 years
- Quality Measures - Clinical outcomes (falls, infections, pressure ulcers, etc.)
- Staffing - Nurse and aide staffing hours relative to residents
Data is updated monthly from the CMS Provider Information dataset. A 5-star rating means the facility performs much above average; 1 star means much below average.
Methodology
Every facility on this hub is identified by its six-digit CMS Certification Number (CCN) and pulled from the CMS Five-Star Quality Rating System, Nursing Home Provider Information (Feb 2026 release). The Overall star is a CMS-calculated composite of three components, Health Inspection, Quality Measures, and Staffing, each published on the same 1–5 scale. Staffing hours come from the payroll-based-journal (PBJ) feed that CMS audits against facility payroll records; deficiency counts are drawn from Rating Cycle 1 (most recent completed standard survey) of the CMS-2567 inspection record.
CMS refreshes the dataset monthly and suppresses any metric that would re-identify individual residents. PlainDoctor publishes these figures exactly as CMS releases them, no custom weighting, no proprietary rankings. See the full methodology for sourcing, pipeline, and limitations.
