What a deactivated NPI means

An NPI identifies who bills. CMS deactivates an NPI for one of a few recorded reasons, such as a practice that closed, and PlainDoctor does not publish the reason. A deactivated NPI cannot be used on a claim. It does not say whether the person still holds a license: that is on the state board's search. A clinician who practises again can have the NPI reactivated, and then it returns to the monthly file.

Figures on PlainDoctor are read from the federal files each page names (CMS NPPES, Medicare Part D, MIPS, Open Payments; the HHS-OIG exclusion list only as counts) and from Census population estimates, with the file date shown beside them.
